This happens if you're on windows vista. The security controls prevent sm from writing to its own folder for some reason. You still see the song because it's in the program cache (or something) but it will get cleared eventually.

If this is the case, the best thing to do is to reinstall Stepmania into My Documents (really!) and things should work fine.
