I just like how clean it looks and also the song banner being used as life frame. My guess is that it might be one of Staiain's themes judging by the style.

It's one of Lily's SM3.95 themes -- I don't exactly remember when she made it (mid 2013 seems about right), but it's an outdated version of the theme (mid 2013?) that you see in videos like Dynam0 or Etienne's later SM3.95 videos (basically most of their videos past mid 2014). There isn't a DL link to it anymore because it was just a previous iteration of a specific theme and wasn't meant to be finalised.
